Information That Must Be Presented When Enrolling at Corner School

• Current Alabama Immunization Record (Check expiration date; kindergarten chicken pox vaccination or proof of chicken pox from doctor; 2nd MMR)

• Certified Birth Certificate

• Social Security Card

• Withdrawal Form and last report card from previous school including complete address of previous school

• Proof of Residence (Total of 3) that include:  1.) Mortgage papers or Bona Fide lease; 2.) two utility bills at that address in the name of the parent or court-appointed legal guardian.  If the lease includes the utilities, provide two (2) of the following items: bank statement, cable bill, tax notice, voter registration card, or page of income tax showing your name and that address.

• Driver's License of the custodial parent

• Current Report Card, Proof of Promotion, or Unofficial Transcript

• Proof of Custody (papers showing custody after divorce or  court-appointed legal guardianship and/or legal name change when applicable.  Neither notarized statements or Temporary Delegation of Parental Rights forms are acceptable.)

• Student Entry/Registration Cards (Can be picked up at school office.)

• Bus Discipline Procedures, Textbook Rules, Internet Usage, Media Center Rules, and Handbook Notification forms (Can be picked up at school office.)

• Certificate of Residency, Home Language Survey, and It's the Law notification forms (Can be picked up at school office.)

• Student Health Record Card for school nurse (Can be picked up at school office.)

• Guidelines for Medication at School for school nurse (Can be picked up at school office.)

• Student Check-Out Authorization Form (Make sure that the names on this form are also on the back of the registration card. Can be picked up at school office.)

• Graduation Exit Exam Brochure (For students in grades 9-12 only.  Can be picked up at school office.)
