This is the form you must use to obtain a Driver's Permit. You MAY NOT download this form, and the Department of Transportation will not accept a fax. You must call our office and request that we print one for you here. We must check your child's attendance records before we can print this form. You may then drop by the High School office to pick this up. |

Fill out this form if you would like to try and qualify for free and reduced benefits. Everyone who wants to apply must fill in Part 1. Homless, migrant and runaway students must fill in Part 2. Foster children only must fill in Part 3. Each Foster child MUST be on a separate application. If you do not have a Food Stamp number, you MUST fill out Part 4, the section on income. |
