Registration Information

Chalkville Elementary School

Dear Chalkville Elementary School Parents,

Summer is around the corner and we want to make sure before you pack and leave that you mark your calendars with the registration dates below.  We will send home registration packets on May 9, 2008 and you will have time to complete the forms and bring them in for processing on the correct date.  Check below for a list of acceptable proofs and other registration information.

Chalkville Elementary School Acceptable List of Proofs

   State of Alabama Immunization Form

  Social Security Card

  Three original proofs of Residence (Current Utility Bills, Mortgage , or lease, which

        indicates the name and address of custodial parent)

  Certificated Birth Certificate (Required for Kindergarten and First grade students)

  Legal Decree of divorce and/or custodial documents, If applicable

  Report card if transferring from another school
